After the confrontation in Hellraiser II, the Cenobite named Pinhead is trapped, along with the puzzle box, in the Pillar of Souls. When the puzzle box is accidentally opened, the evil Pinhead is unleashed, prompting a murderous spree to gain power and to free himself into the real world. When an ambitious reporter, Joey (Farrell), starts to investigate the slayings, she discovers that she is the one who must bring Pinhead and his newly-created Cenobites back to their realm, or else doom herself and the mortal world to an eternity of pain and suffering.
